Questionaires

Every day we receive a large volume of email and telephone inquires regarding employment problems. It is impossible for us to schedule in-person appointments for everyone who calls our office. Over the years we have found the questionnaire to be an efficient device in helping us decide which employment-related cases we can handle.

After your initial phone or email contact with us we may ask you to fill out one of the detailed questionnaire. Please download and print the questionnaire then fill it out as completely as possible. Please do not fill in a questionnaire unless our office has specifically requested you to. Then mail your questionnaire to:

After reviewing your questionnaire, if we believe we can help you with your employment problem, we will send you a letter asking you to schedule an in-person appointment. If we believe we cannot help you, we will notify you of our decision in writing. If possible, we will try to refer you to other attorneys, government agencies, or community organizations that may be able to help you. You may expect a response from our office approximately one week after we receive your completed questionnaire.
